On line Trip Application - Please read and follow these instructions
carefully.
PLEASE NOTE! ! !
TSA requires that the
name on your airline ticket must match EXACTLY, with the name on the ID you will
be using to get through security at the airports. Also required is your date of
birth and your gender. Both will be on your boarding passes and must match
with your ID. Please make sure that you fill in the trip application
properly, so that this information can be passed along to the airline for
ticketing. A minimum $20 charge will be levied by the airline to make
changes to your boarding pass and you could be delayed getting on your flight,
if you have to return to the check in counter to get the changes made.
Before filling in this application, you
should have contacted the Trip Leader and verified that there was space
available on their trip. While contacting the Trip Leader you should have
identified your method of payment, check or credit card and provided the Trip
Leader all pertinent information.
ALSO NOTE:
Submitting this application does not guarantee acceptance to participate on the
trip.
You must get confirmation from the Trip Leader.
Please fill out as completely as
possible.

Premature hitting the 'enter' key will launch the submittal process and if you
have not completed the application, you may have to start over.
Use the helpful
note below to avoid starting over.
Once you have completed the form and clicked submit button, your application will be
processed and the information you provided will appear on the Verification
Page. Print the Verification Page and keep it for your records. You can return
to the trip description page by clicking the link at the bottom of the
Verification Page or hitting your back button until you return to where you want
to be.
Helpful Programming Note:
Should you need to return to your original form because of missing a required
field, using the 'BACK' button from your browser will get you there without
having to reenter all your information.
