On line Volunteer Application - Please read and follow these instructions
carefully.
PLEASE NOTE:
Submitting this application does not guarantee acceptance to participate on a
particular committee.
You will be contacted by the appropriate committee chairperson.
Please fill out as completely as
possible.
Remember, do not type any more information in a single field, than remains
viewable without carriage returns. Use subsequent fields to continue your
descriptions.
Fields with an * are required. Application will not submit otherwise.
Use your tab key or click into each field as you move around the form.
Premature hitting the 'enter' key will launch the submittal process and if you
have not completed the application, you may have to start over. Use the helpful
note below to avoid starting over.
Once you have completed the form and clicked submit button, your order will be
processed and the information you provided will appear on the Verification
Page. Print the Verification Page and keep it for your records. You can return
to the Volunteer Information page by clicking the link at the bottom of the
Verification Page or hitting your back button until you return to where you want
to be.
Helpful Programming Note:
Should you need to return to your original form because of missing a required
field, using the 'BACK' button from your browser will get you there without
having to reenter all your information.
